ZPP is typically found in small amounts in red blood cells. When the iron levels in the blood are low due to iron deficiency or when lead is present in the blood, Zinc combines with protoporphyrin instead of iron. When the protoporphyrins in the red blood cells can not combine with iron, the molecule hemoglobin can not carry oxygen through the body.

In an industrial setting, OSHA strongly recommends that the ZPP test be ordered every time that a lead level is ordered to monitor exposure to lead. It is best to order both a lead blood test and ZPP test as the ZPP test alone will not indicate if the exposure is recent. The ZPP test is best at detecting the average exposure to lead over a period of 3 to 4 months.
