Contest Rules: 1) You must have purchased testing from Health Testing Centers, using the email submitted for this contest to receive test results. 2) You may leave only one testimonial. If you enter more than once you will be disqualified. 3) Employees and their immediate family members, of Health Testing Centers, and its affiliated and subsidiary companies are not eligible to participate in this contest. 4) The winner will receive one (1) new iPad mini (Wi-Fi, 16 GB, Black & Slate) with a value of $329. No cash redemption is permitted. The winner is responsible for all federal, state, and local taxes on the prize. 5) If you are chosen, you will be notified via e-mail. The winner must respond within five days of the end of the contest. If you do not respond within that period, another winner will be chosen. Health Testing Centers will send the prize within fifteen (15) business days of the date the winner responds and provides a valid postal address. 6) Entries can be submitted until the Contest End Date (February 15, 2013 at 12 p.m. ET). 7) You give permission to use the testimonial with your first name and last name initial for Health Testing Centers marketing purposes. 8) You must be legal resident of the United States and at least 18 years of age to enter contest. Contest is void where prohibited by law.
